Dose depends on renal function. It may be appropriate to check peak and trough levels in some patients (e.g. patients with high body weight). Aim for a peak (1 hour post-dose) >10mg/L and trough <1mg/L. For renal patients or synergistic dosing and monitoring of gentamicin in endocarditis, refer to respective guideline on Antimicrobial site – link only active if accessing from NHS network.
