Use patient specific dose in cystic fibrosis (CF). If patient specific dose is not available, use gentamicin dosing calculator/guidelines for initial dosing and monitoring as pharmacokinetics of tobramycin are identical to gentamicin. Contact clinical/antimicrobial pharmacist for advice. |
