Documentation and Governance

• Medical Notes: Ensure the indication, intended duration, and 48-72 hour review outcome (CARES) are clearly documented in the patient record.
• Daily Review: All medical and ANP staff should review the need for IV therapy during daily ward rounds.
• Escalation: Request urgent senior review if the patient deteriorates or shows signs of septic shock.
